Controlling ventilation to prevent increased intracranial pressure is the key idea. The patient’s decerebrate posturing and unequal pupils indicate brainstem injury with likely rising intracranial pressure (evidenced by the Cushing-like vital signs of very high blood pressure with a relatively slow pulse). In traumatic brain injury, hypercapnia (elevated CO2) worsens intracranial pressure by causing cerebral vasodilation, which can push the brain toward herniation. Therefore the priority is to ensure adequate ventilation to maintain normocapnia (roughly a normal PaCO2, around 35–40 mmHg) while keeping the airway protected and oxygenation adequate. A ventilation rate of about 20 breaths per minute provides controlled ventilation to maintain normal CO2 levels without overshooting into hypocapnia, which could also compromise cerebral blood flow. Fluids alone don’t address the elevated ICP, and relying on oxygenation alone without managing CO2 can let ICP rise; rapid transport without airway management risks airway failure in a patient with severe brain injury.
